Double Pylorus in Upper Gastrointestinal Bleeding

Double pylorus, also known as acquired double is a rare condition defined gastrointestinal fistula connecting stomach antrum and duodenal bulb. The prevalence of pylorus ranges from 0.001 to 0.4% by esophagogastroduodenoscopy (EGD). Although the etiology unknown, formation related <i>Helicobacter pylori</i> infection use non-steroidal anti-inflammatory drugs (NSAID). development occurrence still but many systemic diseases play role. We present case 59-year-old man who was admitted Dr. Soetomo General Hospital with hematemesis melena. patient had history diabetes mellitus since 3 years consumption medicinal herbs for myalgia, which suspected NSAIDs past 5 months. anemia hemoglobin at 8.3 g/dL, enterogenous azotemia blood urea nitrogen 28 mg/dL serum creatinine 1.14 mg/dL. At EGD, found accompanied gastric ulcer, giant white base part it covered clotting without any sign active bleeding. Biopsy revealed chronic inactive gastritis, no <i>H. found. Treatment mainly depends on acid suppression through proton pump inhibitor (PPI). given high-dose PPI mucosal protective agent. He treated 1 week improved complaints.
